Clinical Pharmacology

Aktiferrin drops for oral administration is an iron preparation.
Iron is necessary for the life of the body: it is part of hemoglobin, myoglobin, a number of enzymes; reversibly binds oxygen and is involved in a number of redox reactions; stimulates erythropoiesis. The alpha-amino acid serine contained in Aktiferrin contributes to a more efficient absorption of iron and its entry into the systemic circulation, which leads to a rapid restoration of the normal iron content in the body. The inclusion of alpha-amino acid serine reduces the dose of iron, providing better tolerability of the drug.
When using the drug Aktiferrin, there is a rapid replenishment of iron deficiency in the body, which leads to a gradual regression of clinical (weakness, fatigue, dizziness, tachycardia, soreness and dry skin) and laboratory symptoms of anemia.

Indications

Iron deficiency anemia of various etiologies.
Latent deficiency of iron in the body, associated with excessive loss of iron (bleeding) or with an increased need for it (the period of active growth;

Dosage and Administration

Newborns and younger children, the daily dose of Aktiferrin set at the rate of 5 drops / 1 kg of body weight; the multiplicity of appointments - 2-3 times / day.
Average doses:
For infants make up 10-15 drops 3 times / day.
For children of preschool age - 25-35 drops 3 times / day.
For children of school age - 50 drops 3 times / day.
Drops are taken with a small amount of liquid (fruit tea or water).
Treatment with the drug continues for at least 8-12 weeks after reaching normal levels of serum iron and hemoglobin, prescribing a maintenance dose.

Adverse reactions

From the digestive system: rarely, a feeling of heaviness in the epigastric region, flatulence, constipation, or diarrhea, which disappear with dose reduction.

Contraindications

Hemochromatosis, hemosiderosis;

Drug interactions

Antacid agents, calcium preparations, gastric acid reducing agents (including cimetidine, drugs containing carbonates, bicarbonates, phosphates, oxalates), pancreatin, pancreatolipase, etidrone, caffeine, reduce the absorption of Aktiferrin (the interval between taking these medicines funds should be 1-2 hours).

When used together, ascorbic acid increases iron absorption.
Aktiferrin reduces the absorption of fluoroquinolones, penicillamine, tetracyclines (these drugs are recommended to be taken 2 hours before or 2 hours after taking iron supplements).

Aktiferrin in high doses reduces renal absorption of zinc drugs (these drugs are recommended to be taken 2 hours after taking iron supplements).

Ethanol increases iron absorption and increases the risk of developing toxic complications.

Pregnancy and Lactation

With established deficiency of iron in the body during pregnancy and lactation (breastfeeding), use of Aktiferrin is considered reasonable and safe.

Special instructions

With caution prescribed to patients with gastric ulcer and duodenal ulcer, with inflammatory bowel disease (enteritis, diverticulitis, ulcerative colitis, Crohn's disease), chronic alcoholism, allergic diseases, asthma, hepatitis, hepatic and renal insufficiency, rheumatoid arthritis, an art unit. .
Care should be taken when prescribing Aktiferrin to patients with diabetes mellitus, since the drug contains carbohydrates: 18 drops (1 ml) - 64 mg, which is equivalent to 0.15 XE and 0.0053 XE, respectively.
Aktiferrin is not washed down with black tea, coffee, milk in order to avoid a decrease in iron absorption. In addition, solid food, bread, raw cereals, dairy products, eggs can cause a decrease in absorption. The interval between taking Aktiferrin and the use of the listed products should be 1-2 hours.
For the prevention of reversible dark bloom on the teeth of patients, Aktiferrin should not be taken undiluted, after eating it is recommended to thoroughly brush your teeth.

Overdosage

Symptoms: in case of accidental use of the drug in very high doses, weakness, fatigue, paresthesia, pallor of the skin, decreased blood pressure, palpitations, acrocyanosis, abdominal pain, diarrhea mixed with blood, vomiting, cyanosis, confusion, weak pulse, hyperthermia, lethargy are possible. , convulsive seizures, hyperventilation symptoms, coma.

Signs of peripheral vascular collapse appear within 30 minutes after administration; metabolic acidosis, convulsions, fever, leukocytosis, coma - within 12-24 h; acute renal and hepatic necrosis - in 2-4 days.

Treatment: before carrying out specific therapy - gastric lavage, taking milk, raw eggs. Specific therapy is carried out by the prescription of deferoxamine (desferal) orally and parenterally.

For acute poisoning to bind iron, not yet absorbed from the gastrointestinal tract, give 5-10 g deferoxamine inward by dissolving the contents of 10-20 vials in drinking water. To remove absorbed iron, deferoxamine is administered intramuscularly slowly, for children - 15 mg / h, for adults - 5 mg / kg / h (up to 80 mg / kg / day); in case of mild poisoning, i / m children take 1 g every 4-6 hours, adults 50 mg / kg (up to 4 g / day).

In severe cases, accompanied by the development of shock, carried out IV the drip of the drug in a dose of 1 g and spend symptomatic therapy.

Hemodialysis is not effective for the removal of iron, but can be used to accelerate the elimination of the iron-deferoxamine complex, and can also be prescribed for oligo- and anuria. Perhaps peritoneal dialysis.
