Clinical Pharmacology
Terbinafine - the active substance Fungerbin ® NEO, is an allylamine and has a wide spectrum of antifungal action. Terbinafine disrupts the early stages of ergosterol biosynthesis in fungi. This leads to a deficiency of ergosterol and to the intracellular accumulation of squalene, which causes the death of the fungal cell. The action of terbinafine is carried out by inhibiting the enzyme squalene epoxidase, located on the cell membrane of the fungus. This enzyme does not belong to the cytochrome P450 system, therefore, terbinafine does not affect the metabolism of hormones or other drugs. Terbinafine has a fungicidal effect against dermatophytes, molds and some dimorphic fungi. Activity against yeast-like fungi, depending on their species, may be fungicidal or fungistatic. Urea is the second active component of Fungerbin® NEO, has a softening, exfoliating and moisturizing effect, and is able to increase dermatopenetrantness (penetration into the skin) and create high concentrations of terbinafine in the skin. Urea helps to bind water and soften the stratum corneum.
Indications
Treatment and prevention of fungal diseases: Fungal infections of the skin caused by dermatophytes such as Trychophyton (for example, T.rubrum, T.mentagrophytes, T.verrucosum, T.violaceum), Microsporum canis, and Epidermophyton floccosum. Skin infections caused by yeast-like fungi of the genus Candida (for example, Candida albicans). Pityrosporum versicolor, caused by Pityrosporum orbiculare (also known as Malassezia furfur).
Composition
1 g of cream contains: Active substances: Terbinafina hydrochloride in terms of 100% substance - 0.01 g, urea - 0.10 g; Auxiliary substances: Butylhydroxytoluene (dibunol), macrogol cetostearate (Eumulgin® B 2), cetostearilovy alcohol [cetyl alcohol 60%, stearyl alcohol 40%], poloxamer (emuksol-268), imidol (i))))); vaseline medical), purified water - up to 1 g.

Dosage and Administration
Adults and children from 12 years. Before applying the cream must be cleaned and dried the affected areas. The cream is applied 1 or 2 times a day with a thin layer on the affected skin and surrounding areas and lightly rubbed.
For infections involving diaper rash (under the mammary glands, in the interdigital spaces, between the buttocks, in the groin area), the places of application of the cream can be covered with gauze, especially at night. For extensive fungal infections of the body, it is recommended to apply cream in tubes of 30 g each.
The average duration of treatment: ringworm of the trunk, legs - 1 time per day for 1 week; dermatomycosis of the feet - once a day for 1 week; horniness, cracks, itching and peeling of the skin of the foot, caused by a fungus - 1-2 times a day for 2 weeks; skin candidiasis - 1-2 times a day for 1-2 weeks; versicolor versicolor - 1-2 times a day for 2 weeks.
Reducing the severity of clinical manifestations is usually observed in the first days of treatment. In the case of irregular treatment or its premature termination, there is a risk of re-infection. If there are no signs of improvement after 1–2 weeks of treatment, the diagnosis should be verified.
Elderly age. The dosing regimen of lamisil cream in the elderly does not differ from the above.
Children. The use of this drug in children under 12 years is not recommended.
Adverse reactions
Local reactions: redness, itching, or burning.
Other: allergic reactions.
Contraindications
Hypersensitivity to terbinafine or to any of the inactive ingredients that make up the drug.
Overdosage
Symptoms: in case of accidental ingestion of the drug, headache, nausea, epigastric pain and dizziness are possible. The content of ethanol in the spray should also be 23.5%.
Treatment: activated carbon, if necessary, carry out symptomatic and supportive therapy.
