Clinical Pharmacology

Joset - the combined drug, has a bronchodilator, expectorant, mucolytic action.
- Salbutamol is a bronchodilator, stimulates the β2-adrenoreceptors of the bronchi, warns or stops bronchospasm.
- Bromhexine - mucolytic agent, has expectorant and weak antitussive effect. Reduces the viscosity of sputum due to the depolarization of acid mucopolysaccharides and stimulation of the secretory cells of the bronchial mucosa. Activates the cilia of the ciliated epithelium, improving sputum discharge.
- Guaifenezin - reduces the surface tension and adhesive properties of sputum, increases the serous component of bronchial secretions, reduces the viscosity of sputum, activates the ciliary apparatus of the bronchi, facilitates the removal of sputum and promotes the transition of non-productive cough to productive.
- Menthol - has a spasmolytic effect, gently stimulates the secretion of bronchial glands, has antiseptic properties, has a calming effect and reduces irritation of the mucous membrane of the respiratory tract.

Indications

Acute and chronic bronchopulmonary diseases, accompanied by difficulty in sputum discharge:
- bronchial asthma;
- Chronical bronchitis;
- emphysema;
- COPD;
- tracheobronchitis;
- pneumonia;
- pneumoconiosis;
- pulmonary tuberculosis.

Composition

5 ml of syrup contains: salbutamol sulfate 1.205 mg, which corresponds to the content of salbutamol 1 mg, bromhexine hydrochloride 2 mg, guaifenesin 50 mg, menthol (levomenthol) 500 mcg,
Excipients: sodium methyl parahydroxybenzoate, sodium propyl parahydroxybenzoate, sucrose, propylene glycol, sodium saccharinate, sorbic acid, glycerol, citric acid monohydrate, sorbitol (non-crystallizable) liquid, dye sunset-yellow, purified water.

Dosage and Administration

The drug should be taken orally.
Children under the age of 6 years - 1 tsp. (5 ml) 3 times / day; from 6 to 12 years old - 1-2 tsp. (5-10 ml) 3 times / day.
Adults and children over 12 years old - 2 tsp. (10 ml) 3 times / day.

Adverse reactions

From the side of the central nervous system: headache, dizziness, increased nervous irritability, sleep disturbance, drowsiness, tremor, convulsions.
On the part of the digestive system: nausea, vomiting, diarrhea, exacerbation of gastric ulcer and duodenal ulcer; very rarely - increased activity of hepatic transaminases.
Since the cardiovascular system: tachycardia, lower blood pressure, collapse.
On the part of the urinary system: pink staining of urine.
Allergic reactions: skin rash, urticaria, paradoxical bronchospasm.

Contraindications

- tachyarrhythmia;
- myocarditis;
- aortic stenosis;
- decompensated diabetes;
- thyrotoxicosis;
- glaucoma;
- liver and / or renal failure;
- gastric ulcer and duodenal ulcer in the acute phase;
- gastric bleeding;
- pregnancy;
- lactation period (breastfeeding);
- Hypersensitivity to the components of the drug Djoset.
With caution should use the drug for diabetes, hypertension, gastric ulcer and duodenal ulcer history; simultaneously with antitussives, non-selective β-adrenoreceptor blockers, MAO inhibitors.

Drug interactions

Beta2-adernomimeticheskie means, theophylline and other xanthines, while the use of the drug increases the likelihood of developing tachyarrhythmias.
MAO inhibitors and tricyclic antidepressants increase the effects of salbutamol, which can lead to a sharp decrease in blood pressure.
Diuretics and GCS enhance the hypokalemic effect of salbutamol.
The simultaneous use of drugs containing codeine, and other antitussive agents complicates the discharge of liquefied sputum.
Part of the drug Bromhexine contributes to the penetration of antibiotics into the lung tissue.
Not recommended for use simultaneously with non-selective blockers of β-adrenergic receptors.

Special instructions

Guaifenesin paints urine pink.
It is not recommended to drink the drug alkaline drink.

Overdosage

Symptoms: increased manifestations of side effects.
Treatment: symptomatic therapy.
