Expal has a laxative effect.

Pharmacodynamics

In the large intestine, lactitol is cleaved by the action of intestinal flora into low molecular weight organic acids, which leads to an increase in osmotic pressure in the large intestine, an increase in the amount of feces, their softening, easier defecation and normalization of the intestine. The laxative effect usually occurs within 24 hours after the use of the drug (the delay is due to the passage of the drug through the gastrointestinal tract). At the beginning of the course of treatment, a laxative effect of the drug may be delayed, the effect may occur on the 2nd or 3rd day of its use.

In patients with hepatic encephalopathy or coma (or precoma), the effect is achieved by migration of ammonia from the blood into the colon (due to lower pH, increased osmotic pressure in the intestinal lumen), removal of retained ammonium ions and other nitrogen-containing toxic substances from the colon. Lactitol inhibits proteolytic bacteria and increases the number of acidophilic bacteria.

Pharmacokinetics

Lactitol carries out its effect only in the large intestine, where it completely falls after ingestion. Absorbed in minimal quantities in unchanged form (no more than 0.5–2% of the accepted dose).

Indications

  • constipation, intestinal dysbiosis;
  • the need for stool regulation for medical purposes (intestinal cleansing in preparation for endoscopic and X-ray examinations, surgical interventions on the rectum, anal sphincter and the area adjacent to it);
  • hepatic encephalopathy, hepatic precoma and coma, hyperammonemia.

Dosage and Administration

For oral use while eating, mixing with various drinks (including water, tea, coffee, juices) or liquid foods.

Constipation and dysbacteriosis. The drug should be taken once, the entire daily dose at once, at one time.Adults- 20 g / day (4 tsp. Of powder);children 12–16 years old - 10–20 g / day (2–4 tsp. Of powder);6-12 years old - 5–10 g / day (1–2 tsp. Of powder);1–6 years old - 2.5–5 g / day (0.5–1 h. Spoon of powder).

The drug should cause stool 1 time per day; after the onset of a stable laxative effect within a few days of using the drug, its dose can be reduced. In some cases, to obtain the desired effect is enough half the recommended daily dose. The minimum course of therapy, after which you should consult a doctor, is 5 months. The use of the drug in children is recommended under the supervision of a physician.

Regulation stool for medical purposes. 20 g of the drug, diluted in 1 liter of water, are taken 3 times (at 12, 15 and 18 hours) on the eve of medical procedures.

Hepatic encephalopathy, hepatic precoma and coma, hyperammonemia. The dose of the drug should be selected individually for each patient, depending on the severity of the disease and the individual reaction to the drug. The initial daily dose is usually 0.5–0.7 g / kg. This dose is divided into 3 single doses. Should be selected dose, causing two soft chairs per day.

Adverse reactions

At the beginning of treatment, a feeling of discomfort in the abdomen, flatulence. These phenomena, as a rule, disappear with continued use of the drug, as it adapts to it.

In the case of the use of higher doses for a long time in the treatment of hepatic encephalopathy in a patient, due to diarrhea, an electrolyte imbalance may develop.

Contraindications

  • individual intolerance to lactitol;
  • galactosemia;
  • intestinal obstruction;
  • suspected organic damage to the digestive tract;
  • abdominal pains;
  • rectal bleedings of not clear genesis.

Drug interactions

While taking the drug Expal® with enteric pH-dependent release preparations, it should be taken into account that lactitol lowers the pH in the intestine. It is not recommended to use Exportal® within 2 h after taking other drugs.

Special instructions

Lactitol has a low calorie (2 kcal / g), does not affect the level of glucose in the blood and can be used in patients with diabetes.

Throughout the course of the drug is preferably increased fluid intake.

Influence on ability to steer vehicles and work with mechanisms.Use of the drug Expal® does not affect the psychomotor functions associated with driving a car or driving machinery.

Symptoms: diarrhea; in rare cases, vomiting.
