Clinical Pharmacology

Lorinden A is a combined preparation with anti-inflammatory and keratolytic effects for external use.

Flumetazon pivalate is a synthetic GCS, has anti-inflammatory, anti-edematous, anti-allergic, antipruritic action. Inhibits the activity of phospholipase A 2, which leads to the suppression of the synthesis of prostaglandins and leukotrienes, inhibits the release of inflammatory mediators. It inhibits the migration of leukocytes and lymphocytes to the inflammatory focus. Prevents regional accumulation of neutrophils, which leads to a decrease in inflammatory exudate, cytokine production and inhibition of macrophage migration, resulting in reduced processes of infiltration and granulation. Inhibits the proteolytic activity of tissue kinin, retards the growth of fibroblasts, prevents the development of connective tissue in the inflammation. Reduces hyperemia, manifestations of hypersensitivity reactions, proliferative and exudative processes occurring in the connective tissue in the focus of inflammation.

Salicylic acid - NSAIDs, contributes to the penetration of GCS and gives the drug additional antiparakeratosis, moderate keratolytic and local hypothermic properties, contributes to the penetration of flumethasone into the skin. In addition, it has antibacterial and fungicidal action, and also restores the protective function of the skin. Suppresses the secretion of sebaceous and sweat glands.

Indications

- Acute and chronic allergic dermatosis, especially accompanied by excessive keratinization of the skin:
- Atopic dermatitis.
- Diffuse atopic dermatitis.
- Chronic lichen Vidal.
- Various forms of subacute and chronic eczema (especially horny).
- Hyperkeratosis (for example, ichthyosis).
- Chronic dyshidrosis.
- Psoriasis.
- Seborrhea.
- Red lichen planus.
- Red wart lichen.
- As part of combination therapy for cystic skin diseases (including cystic eruptions on the skin of the palms and feet).

Composition

1 g of ointment for external use contains: Flumethasone Pivalate 200 mcg, Salicylic Acid 30 mg.

Excipients: propylene glycol, anhydrous lanolin, white petrolatum.

Dosage and Administration

The ointment is applied with a thin layer on the skin in the affected area 2-3 times / day. After removal of acute inflammation, the drug is used 1-2 times / day. After the complete disappearance of the painful manifestations, the treatment is carried out for another 3-4 days. For chronic skin lesions, treatment should not be continued for more than 3 weeks.

The ointment can also be used as an occlusive dressing, which should be changed every 24-48 hours. In the treatment of lesions on dry and degreased skin areas characterized by severe infiltration, lichenisation, desquamatization and hyperkeratosis, as well as mozolelostyu, the degree of moisture can be adjusted using the thickness applied layer, in accordance with the therapeutic requirements characteristic of each individual case.

Adverse reactions

Local reactions: possible - burning, itching, dry skin; with prolonged use - atrophy of the skin, local hirsutism, telangiectasia, purpura, steroid acne, perioral dermatitis, pigmentation disorders.
Systemic reactions: long-term use, application to extensive skin areas and / or the use of occlusive dressings may have side effects characteristic of GCS. With external use of the drug Lorinden A on the skin of the eyelids in the future, sometimes, the development of cataracts or glaucoma is possible.

Contraindications

- Bacterial skin diseases.
- Viral diseases of the skin (including chicken pox, shingles).
- Fungal diseases of the skin.
- Acute weeping and subacute exudative stages of skin diseases.
- Tuberculosis of the skin.
- Skin manifestations of syphilis.
- Skin neoplasm.
- Pre-cancerous conditions of the skin.
- Vulgar and rosacea.
- Trophic ulcers of the legs associated with varicose veins.
- I trimester of pregnancy.
- Infants and young children.
- Hypersensitivity to the drug.

Drug interactions

During treatment with a drug, vaccination and immunization are contraindicated due to the immunosuppressive effect of the drug.

It should not be prescribed in combination with other drugs for external use.

With systemic absorption of the drug GCS reduces the effect of insulin, oral hypoglycemic agents, antihypertensive drugs, anticoagulants, reduces the concentration of praziquantel in blood serum.

It increases the risk of side effects, combination Lorinden A with the following drugs: androgens, estrogens, oral contraceptives, anabolic steroids (hirsutism, acne), antipsychotics bucarban, azathioprine (cataract) holinoblokatory, antihistamines, tricyclic antidepressants, nitrates (glaucoma ), diuretics (hypokalemia), cardiac glycosides (digitalis intoxication).

Pregnancy and Lactation

The use of the drug is contraindicated in the first trimester of pregnancy.

Use of the drug during pregnancy is possible as an exception, on limited skin areas and only in the case when the intended benefit to the mother outweighs the potential risk to the fetus.

The use of the drug during lactation (breastfeeding) is possible in exceptional cases, briefly, in limited areas of the skin; use of the drug on the skin of the mammary glands is contraindicated.

Special instructions

Lorinden A is an adequate drug for the treatment of lesions on dry and degreased skin areas characterized by severe infiltration, lichenisation, desquamation and hyperkeratosis, as well as mozozheliele; it is easy to apply, it adheres well to dry skin, increases its fat content and helps retain moisture. The degree of moisture can be adjusted using the thickness of the applied layer, in accordance with the therapeutic requirements characteristic of each individual case.

It is contraindicated to apply the drug on the skin.It is necessary to ensure that the drug does not fall on the mucous membranes and conjunctiva.

It is not recommended to exceed the course of treatment. With prolonged use of the ointment on large areas of the skin, the frequency of side effects increases.

When an infection occurs at the site of application of the ointment, it is necessary to apply agents with a more pronounced antibacterial or antifungal effect.

Caution should be used in patients with atrophic skin changes, especially in elderly patients.

The ointment does not contaminate clothing and bedding.

- Use in cases of impaired renal function. Repeated application over large areas of skin in patients with severe renal insufficiency should be avoided (systemic action of salicylic acid is possible).

- Use in pediatrics In children, the drug is used only in case of absolute necessity, in a short course, on small body surfaces.

- Impact on the ability to drive vehicles and control mechanisms The drug does not limit the psychophysical activity, the ability to drive vehicles and maintain moving mechanisms.

Overdosage

Symptoms: extremely rarely, when applied on very large areas of the skin, systemic manifestations of GCS and / or symptoms of salicylate poisoning are possible.
