Clinical Pharmacology
Maxitrol is a combined preparation for topical use in ophthalmology. Glucocorticosteroid + antibiotics (aminoglycoside and cyclic polypeptide).
Neomycin
It has a bactericidal effect, disrupting bacterial cell protein synthesis. Active against: Staphylococcus aureus, Corynebacterium diphteriae, Streptococcus viridans, Escherichia coli, Klebsiella pneumoniae, Proteus vulgaris, Aerobacter aerogenes, Haemophilus influenzae.
Polymyxin
The mechanism of action of polymyxin B is mainly due to the blockade of the permeability of the cytoplasmic membrane of bacterial cells. Active against: Pseudomonas Aeruginosa, Aerobacter aerogenes, Escherichia coli, Klebsiella pneumoniae, Koch-Weeks bacillus.
Dexamethasone
Glucocorticosteroid. It has anti-inflammatory, anti-allergic and desensitizing effects. It has antiexudative effect. Dexamethasone effectively suppresses inflammatory processes.
The combination of glucocorticosteroid with antibiotics reduces the risk of infection.
Indications
Infections of the eye and its appendages:
- Blepharitis
- Conjunctivitis.
- Keratoconjunctivitis.
- Blepharoconjunctivitis.
- Keratitis
- Iridocyclitis.
Prevention of postoperative infectious complications.
Composition
1 ml of solution contains:
Active ingredients
- Dexamethasone 1.0 mg.
- Neomycin sulfate 3500 IU.
- Polymyxin B sulfate 6000 IU.
Excipients
Benzalkonium chloride solution, equivalent to benzalkonium chloride; sodium chloride, polysorbate 20, hypromellose * (4000 mPa.s), concentrated hydrochloric acid and / or sodium hydroxide to bring the pH, purified water.

Dosage and Administration
Locally.
With a mild infectious process
1-2 drops in the conjunctival sac 4-6 times a day.
In acute severe infectious process
1-2 drops to the conjunctival sac every 60 minutes, with a decrease in the frequency of drug installations as the inflammation decreases.
Adverse reactions
Local Allergic reaction, accompanied by itching and swelling of the eyelids, conjunctival hyperemia. Side effects caused by the steroid component: increased intraocular pressure with possible subsequent development of glaucoma, damage to the optic nerve and visual fields (using the drug for more than 10 days requires monitoring of intraocular pressure); rear subcapsular cataract, slowing the healing process of wounds. Topical use of steroids for diseases that cause thinning of the cornea or sclera, can lead to their perforation.
Secondary infection. The development of secondary infection is observed after the use of combined drugs containing GCS in combination with antibiotics. With prolonged use of steroids may develop fungal infections of the cornea. The appearance of non-healing ulcers on the cornea after long-term treatment with steroid preparations may indicate the development of fungal invasion. Secondary bacterial infection may occur as a result of suppression of the protective reaction of the patient.
Contraindications
Caution is prescribed for:
- Glaucoma.
- Cataract
Drug interactions
Interaction with other drugs is currently unknown.
In case of use with other local ophthalmologic preparations, the interval between their use should be at least 10 minutes.
Pregnancy and Lactation
Sufficient experience in the use of the drug during pregnancy and lactation is not.
Perhaps the use for the treatment of pregnant women as prescribed by the attending physician, if the expected therapeutic effect exceeds the risk of possible side effects.
It is necessary to interrupt the use of the drug for the period of breastfeeding.
