Clinical Pharmacology

Occumethyl is a combination drug containing zinc sulfate, diphenhydramine and naphazoline.

Zinc sulfate when applied topically, it has astringent, drying, anti-inflammatory and antiseptic effects.

Naphazoline - sympathomimetic. It has a fast, pronounced and prolonged vasoconstrictor effect. When applied topically, it reduces swelling and hyperemia of the mucous membranes.

Diphenhydramine - blocker H1- histamine receptors. When applied topically, it reduces the permeability of the capillaries and prevents tissue swelling.

Thus, Occumethyl has an antiseptic, antiallergic and anti-inflammatory effect.

Indications

  • Chronic nonspecific conjunctivitis and blepharoconjunctivitis.
  • Allergic conjunctivitis and blepharoconjunctivitis.
  • Angular conjunctivitis.
  • Reducing symptoms of eye tissue irritation (redness, itching, foreign body sensation).

Composition

10 ml of solution contain:

Excipients

Sodium citrate, citric acid, benzalkonium chloride, blue methylene dye, sodium chloride, hypromellose 4000, sodium hydroxide, distilled water.

Dosage and Administration

Locally.

If there are no other instructions of the doctor, then bury 1 drop in the conjunctival sac of the affected eye (s) 2-3 times a day.

If during the use of the drug for 72 hours no reduction of symptoms is observed, the use of the drug should be discontinued and consult a doctor.

Adverse reactions

  • After instillation, a feeling of slight burning, blurring of vision, the development of allergic reactions may occur.
  • When aphakia may develop maculopathy and the emergence of a central scotoma. In this case, withdrawal of the drug leads to the reverse development of symptoms.
  • After local use of naphazoline, which is part of Occumethyl, reactive hyperemia and edema of the mucous membrane of the eyes, dilated pupil, increased IOP (intraocular pressure) may occur.
  • With prolonged use, the phenomenon of tissue irritation occurs most often.
  • Since the local application of naphazoline has a general resorptive effect, it is possible to develop tachycardia, increase blood pressure, nausea, headache.
  • With the local application of diphenhydramine, side effects of both local and systemic nature may develop, paresis of accommodation, photosensitivity, dryness and numbness of the eye mucosa, drowsiness, weakness, decreased psychomotor reactions and impaired coordination of movements, Dizziness.
  • With prolonged use of zinc salts may cause dryness of the mucous membrane of the eyes.

Contraindications

Apply with caution in patients taking MAO inhibitors or other drugs that increase blood pressure.

Drug interactions

Since it is possible to reduce the effects of diphenhydramine and drugs that stimulate the central nervous system, it is not recommended to use Occumethyl in patients using these drugs.

Occumethyl should not be used simultaneously with MAO inhibitors and for 10 days after stopping their administration.

As a result of the joint use of beta-blockers, MAO inhibitors with adrenomimetics, the action of the latter is enhanced.

Naphazoline slows down the absorption of local anesthetics, therefore, combined use of the drug Occumethyl with local anesthetics used in ophthalmology is not recommended.

Zinc sulfate is not pharmaceutically compatible with salts of silver, lead, quinine, ichthyol, citral, protargol, alkaline reactive substances.

Pregnancy and Lactation

Contraindicated in pregnancy and lactation.

Special instructions

To avoid contamination of the solution, keep the vial tightly closed and avoid contacting the pipette tip with any surface.

Influence on ability to drive motor transport and control mechanisms

Within 10-15 minutes after instillation, visual acuity may decrease, and therefore care must be taken when driving and working with machinery.
