Clinical Pharmacology

Calcium-phosphorus metabolism regulator used in systemic osteoporosis.
The drug has a dual effect on bone metabolism: a stimulating effect on osteoblasts and an inhibitory effect on osteoclasts. Osteogenon contains the necessary components for the synthesis of bone tissue.
Calcium, which is part of the drug, is contained in the form of hydroxyapatite (in the ratio of phosphorus 2: 1), which contributes to a more complete absorption of calcium from the gastrointestinal tract; in addition, it inhibits the production of parathyroid hormone and prevents hormone-related bone resorption. Slow release of calcium from hydroxyapatite causes the absence of a peak of hypercalcemia.
Phosphorus involved in the crystallization of hydroxyapatite, promotes calcium fixation in the bone and inhibits its excretion by the kidneys.
The organic component of the drug (ossein) contains local regulators of bone tissue remodeling (transforming growth factor - beta (b-TGF), insulin-like growth factors I, II (IGF-I, IGF-II), osteocalcin, collagen of the first type), which activate the process bone formation and inhibit bone resorption. b-TGF stimulates the activity of osteoblasts, increases their number, contributes to the production of collagen, and also inhibits the formation of osteoclast precursors. IGF-I and IGF-II stimulate the synthesis of collagen and osteocalcin. Osteocalcin promotes bone crystallization by binding calcium. Collagen of the first type provides the formation of bone matrix.

Indications

- prevention and treatment of osteoporosis of various etiologies, including primary (premenopausal, menopausal, senile) and secondary (caused by rheumatoid arthritis, liver and kidney diseases, hyperthyroidism, hyperparathyroidism, imperfect bone formation, and the use of corticosteroids, heparin, immobilization);
- correction of osteopenia and calcium-phosphorus balance disorders (including during pregnancy and during breastfeeding);
- accelerated healing of bone fractures.

Dosage and Administration

Withosteoporosis adults appoint inside on 2-4 tab. 2 times / day.
For other indications, the recommended dose is 1-2 pills / day.
The duration of treatment is determined by the patient's condition.

Adverse reactions

Seldom: allergic reactions.

Contraindications

- hypercalcemia;
- severe hypercalciuria;
- Hypersensitivity to the drug.

Drug interactions

When used together, Osteogenon slows down the absorption of iron preparations and antibiotics of the tetracycline group (the interval between doses should be at least 4 hours).

Pregnancy and Lactation

Perhaps the use of Osteogenon during pregnancy and lactation (breastfeeding) according to indications.
When used during pregnancy, the drug has no damaging effect on the fetus.

Special instructions

In patients predisposed to urolithiasis, correction of the dosage regimen is necessary.
In patients with impaired renal function, prolonged use of the drug in high doses should be avoided.
The preparation contains only trace amounts of sodium chloride, therefore, it is acceptable to use Osteogenon in patients with arterial hypertension.

Overdosage

Treatment of asymptomatic hypercalcemia (see section "Dosage and administration").
Since Rocaltrol® is a vitamin D derivative, it is characterized by the same symptoms of overdose. Taking large doses of calcium and phosphate at the same time as Rocalrol® can cause similar symptoms. The product of serum calcium and phosphorus concentration (CaxP) should not exceed 70 mg2 / dl2. The development of hypercalcemia can contribute to the high calcium content in dialysate.
Symptoms of acute vitamin D poisoning: anorexia, headache, vomiting, constipation.
Symptoms of chronic poisoning: dystrophy (weakness, weight loss), sensitivity disorders, fever with thirst, polyuria, dehydration, apathy, growth retardation and urinary tract infections. The consequences of hypercalcemia are focal calcification of the cortical layer of the kidneys, myocardium, lungs and pancreas.
Treatment of accidental overdose: immediate gastric lavage or taking drugs that cause vomiting, to prevent further absorption. For removing the drug from the feces as a laxative apply liquid paraffin. It is recommended to re-determine the level of calcium in serum. If elevated serum calcium is maintained, you can prescribe phosphates and corticosteroids, as well as take measures to ensure sufficient diuresis.
