Clinical Pharmacology

PENTAMINE - ganglion blocker. Blocks the n-cholinergic receptors of the sympathetic and parasympathetic ganglia, carotid glomeruli and adrenal chromaffin tissue. Interrupts the conduction of nerve impulses through the vegetative ganglia and alters the functions of organs that have vegetative innervation. Reduces the flow of vasoconstrictor pulses to the vessels and the release of adrenergic substances by the adrenal glands, weakens the reflex pressor reactions. It causes the expansion of peripheral vessels, hypotension, tachycardia, mydriasis, violates the accommodation of the eye, expands the bronchi, reduces the motility of the digestive tract, glandular secretion, bladder tone.

Indications

Hypertensive crisis, pulmonary and brain edema, controlled hypotension (in anesthesiology), peripheral vascular spasm, incl. endarteritis, eclampsia, gastric ulcer and duodenal ulcer, smooth muscle spasm of the intestine, gall bladder, bronchus, urinary tract (renal colic), causalgia; in urological practice during cystoscopy in men.

Pentamine

Dosage and Administration

IV, slowly under the control of blood pressure and general condition, in hypertensive crisis, pulmonary edema or brain 0.2-0.5 ml of 5% solution, diluted in 20 ml of isotonic sodium chloride solution or 5% glucose solution, or IM to 2 ml of 5% solution. For controlled hypotension, intravenous drip 0.8-1.2 ml of a 5% solution (0.04-0.06 g) before surgery, if necessary 2.4-3.6 ml of a 5% solution (0.12 -0.18 g). IM with spasms of blood vessels and other non-recurring conditions 1 ml of 5% solution, if necessary, 1.5-2 ml 2-3 times a day. Maximum doses for i / m administration: single 0.15 g (3 ml of 5% solution), daily 0.45 g (9 ml of 5% solution).

Adverse reactions

On the part of the cardiovascular system: increased heart rate, orthostatic collapse (due to the inhibition of reflex mechanisms that maintain a constant level of blood pressure) are possible.

On the part of the digestive system: dry mouth; when used for a long time in high doses, atony of the intestine is possible (up to paralytic ileus of the intestine).

From the side of the central nervous system: possible general weakness, dizziness.

On the part of the organ of vision: mydriasis, accommodation paresis, vascular injection of the sclera.

On the part of the urinary system: when used for a long time in high doses, atony of the bladder with anuria is possible.

Contraindications

Hypersensitivity, degenerative changes in the central nervous system, arterial hypotension, thrombophlebitis, renal and / or liver failure.

Drug interactions

With the simultaneous use of ganglioblokaty increase the effect of adrenergic mimetics.

Anticholinesterase drugs are ganglioblocker antagonists.

With simultaneous use with tricyclic antidepressants decreases the hypotensive effect of ganglioblokatorov.

Pregnancy and Lactation

Special instructions

Azametonium bromide is used with caution in bronchial asthma with difficult sputum, intestinal atony and bladder of various etiologies, thrombosis, thrombophlebitis, degenerative diseases of the central nervous system, bronchial asthma (with difficulty in sputum discharge) in elderly patients.

Treatment with ganglioblockers should be carried out under close medical supervision.

In order to avoid the development of orthostatic collapse, it is recommended that the patient be in a prone position before the introduction of azametonium bromide and for 1-2 hours after the injection. At the beginning of treatment, it is recommended to check the patient's response to small doses of azametonium bromide: 1/2 of the average dose is administered and the patient’s condition is monitored. It should be borne in mind that with increased blood pressure hypotensive effect is more abrupt.

In open-angle glaucoma, a decrease in intraocular pressure may be observed, which is caused by a decrease in the production of aqueous humor under the influence of ganglioblockers.
