Clinical Pharmacology

Rimantadine is an antiviral agent derived from adamantane. The main mechanism of antiviral action is the inhibition of the early stage of specific reproduction after the virus enters the cell and prior to the initial transcription of RNA. Pharmacological efficacy is ensured by inhibiting the reproduction of the virus in the initial stage of the infection process.

Rimantadine is active against various strains of influenza A virus (especially A2 type), as well as tick-borne encephalitis viruses (Central European and Russian spring-summer), which belong to the group of arboviruses of the Flaviviridae family.

Indications

Prevention and early treatment of influenza in adults and children over 7 years old, prevention of influenza during epidemics in adults, prevention of tick-borne encephalitis of viral etiology.

Composition

1 tablet contains rimantadine hydrochloride 50 mg.

Dosage and Administration

Rimantadine should be taken orally after a meal: swallow pills / capsules whole and drink plenty of water.
Recommended doses:

Adults and teenagers older than 14 years: on the 1st day - 100 mg (1 capsule or 2 pills) 3 times a day, 2-3 days - 100 mg 2 times a day, 4-5th days - 100 mg 1 time per day. On the first day of the disease a single dose of rimantadine in a dose of 300 mg (3 capsules or 6 pills) is allowed;
Children 11-14 years: 50 mg (1 tablet) 3 times a day;
Children 7-10 years: 50 mg 2 times a day.

The duration of treatment is 5 days. It is recommended to start taking the drug immediately after the first symptoms of flu appear. Rimantadine is most effective if treatment is started within the first 24-48 hours.

Older people, patients with liver failure and chronic renal failure are prescribed 100 mg 1 time per day.

For the prevention of influenza, adults and children over 7 years old are prescribed 1 tablet of rimantadine 1 time per day for 10-15 days.

Adverse reactions

Rimantadine is generally well tolerated. In rare cases, the following side effects occur:

On the part of the central nervous system: neurological reactions, insomnia, headache, impaired concentration, dizziness, anxiety, drowsiness, fatigue, increased irritability;
On the part of the gastrointestinal tract: loss of appetite, anorexia, dry mouth, flatulence, epigastric pain, nausea, vomiting;
Allergic reactions: itching, rash, urticaria;
Other: hyperbilirubinemia.

Contraindications

Carefully:

Epilepsy, in the history including;
Arterial hypertension;
Diseases of the gastrointestinal tract;
Liver failure;
Chronic renal failure;
Atherosclerosis of cerebral vessels.

Drug interactions

Cimetidine reduces clearance of rimantadine by 18%, paracetamol reduces its maximum concentration in blood plasma by 11%. Urine-defining agents (for example, ascorbic acid and ammonium chloride) accelerate the excretion of rimantadine by the kidneys, thereby reducing its effectiveness. Alkalizing urine drugs (for example, sodium bicarbonate and acetazolamide), on the contrary, reduce the excretion of rimantadine by the kidneys, thereby enhancing its effect. Binding, adsorbing and enveloping agents reduce the absorption of rimantadine. Rimantadine reduces the effectiveness of simultaneously used antiepileptic drugs.

Special instructions

During treatment with rimantadine, there is a possibility of exacerbation of concomitant chronic diseases. In elderly people with hypertension, the risk of hemorrhagic stroke increases, in patients with a history of epilepsy or at the same time receiving anticonvulsant therapy, the risk of developing an epileptic seizure. In the latter case, the daily dose of rimantadine should not exceed 100 mg. Prophylactic administration of the drug is effective at high risk of developing a disease during an epidemic of influenza, contact with the sick, or the spread of infection in closed groups. With influenza B virus, rimantadine has an antitoxic effect. Perhaps the emergence of drug-resistant viruses. Due to the likelihood of side effects on the part of the central nervous system, it is recommended to take caution when treating vehicles and performing potentially hazardous types of work requiring increased attention, high speed mental and / or physical reactions.

Overdosage

The main symptoms of rimantadine overdose are arrhythmia, hallucinations, nervous agitation. In this case, gastric lavage is performed and symptomatic therapy is prescribed. It is also recommended to resort to activities aimed at maintaining vital functions. Rimantadine is partially excreted by hemodialysis.
