Clinical Pharmacology

Acetylcysteine - mucolytic. Thins mucous and purulent-mucous discharge. The mechanism of action is associated with the ability of acetylcysteine sulfhydryl groups to break the disulfide bonds of acidic mucus glycoproteins. It also has anti-inflammatory (due to inhibition of leukocyte chemotaxis) and antioxidant effect.
Tuaminoheptana sulfate - sympathomimetic amine. When applied topically, it has a vasoconstrictor effect, eliminates swelling and hyperemia of the nasal mucosa and sinuses.
Indications
- Acute and subacute rhinitis with a thick purulent-slimy secret;
- chronic rhinitis;
- vasomotor rhinitis;
- sinusitis.
Composition
100 ml of solution contain:
Active substances: acetylcysteine 1 g; tuaminoheptana sulfate 0.5 g;
Excipients: benzalkonium chloride, methyl hydroxypropyl cellulose, disodium edetate, sodium dihydrogen phosphate monohydrate, disodium phosphate dodecahydrate, dithiothreitol, sorbitol 70%, mint flavor, ethanol 96%, sodium hydroxide, purified water.

Dosage and Administration
The drug is spayed into the nasal cavity in the form of an aerosol using a special dispenser.
- Adults: Inject 2 doses of aerosol (2 presses on the valve) into each nostril 3-4 times / day.
- Children: the drug is prescribed in 1 dose (1 click on the valve) 3-4 times / day.
The duration of therapy should not exceed 7 days.
Adverse reactions
Since the cardiovascular system: heartbeat, tachycardia, arterial hypertension.
From the side of the central nervous system: excitement, tremor.
Local reactions: dry nasal, oral and pharyngeal mucosa.
Other: allergic reactions, urinary retention, inflammation of the sebaceous glands.
Contraindications
Carefully: children under 3 years old; with bronchial asthma; with hypertension; with angina III-IV functional classes; with frequent extrasystoles (treatment should be under medical supervision).
Drug interactions
Not compatible with MAO inhibitors and tricyclic antidepressants, as the latter potentiate the action of sympatomime substances, and this can lead to the development of systemic side effects due to tuaminoheptan.
With the simultaneous use of rinofluimucil with antihypertensive agents, a weakening of the antihypertensive effect is possible.
Pregnancy and Lactation
Use of the drug during pregnancy and lactation is possible only in cases where the intended benefit to the mother outweighs the potential risk to the fetus and child.
Special instructions
The drug is intended only for intranasal administration.
With prolonged use of vasoconstrictor agents, the normal function of the mucous membrane of the nasal cavity and sinuses may change, as well as the development of addiction to the drug.
Do not exceed the recommended dose and course of treatment without consulting a doctor.
Impact on the ability to drive vehicles and other mechanisms that require high concentration of attention
The drug does not affect the ability of patients to engage in activities requiring increased attention.
Overdosage
Symptoms: possible development of systemic side effects due to Tuaminoheptan contained in the drug (tachycardia, tremor, anxiety, increased blood pressure, addiction).
Treatment: symptomatic therapy is carried out if necessary.
