Clinical Pharmacology

Risendros® pills - an inhibitor of bone resorption.

Risedronic acid binds to hydroxyapatite at the cellular level and inhibits the function of osteoclasts, reduces bone resorption.

The time to achieve the maximum therapeutic effect is 3 to 6 months, the duration of the therapeutic effect is 12 months.

Indications

Treatment of established postmenopausal osteoporosis.

Composition

1 tablet contains: Risedronate sodium 35 mg.

Excipients: Core - microcrystalline cellulose, crospovidone, magnesium stearate

Shell composition: Hypromellose 2910/5, talc, macrogol 6000, titanium dioxide, iron dye red oxide (E 172), iron dye yellow oxide (E 172)

Risendros® [Risendronic acid]

Dosage and Administration

Recommended dose for adults: 35 mg once a week.

The pill must be taken on the same day of the week.

Risedronic acid absorption depends on food intake, therefore, to ensure adequate absorption, patients should take Risendros® no less than 30 minutes before the first meal, another drug or drink (other than water).

Patients need to be explained that if they missed a dose, it is necessary to take the drug on the day they remembered it. Then the patient should return to taking one pill once a week on a regular day.

You should not take two pills on the same day.

The tablet should be taken whole and not chewed, it is better to stand and drink with plain water (> 120 ml). After taking the pill, patients should not go to bed for 30 minutes.

An adequate diet with sufficient calcium and vitamin D is recommended during treatment. If necessary, additional administration of calcium and vitamin D supplements is possible.

Adverse reactions

On the part of the digestive system: often - dyspepsia, nausea, gastralgia, abdominal pain, constipation, diarrhea; sometimes - esophagitis, ulcers of the esophagus and stomach, gastritis, colitis, dysphagia, belching, duodenitis, glossitis, stricture of the esophagus, a violation of indicators of liver function.
Nervous system disorders: headache, dizziness, asthenia, fatigue, calf muscle cramps, myasthenia.
On the part of the musculoskeletal system: osteomuscular pain, cramps of the gastrocnemius muscles.
From the senses: acute iritis (scleral hyperemia, eye pain), amblyopia, blurred vision, dry eye mucosa, tinnitus.
Allergic reactions: skin rash.
Others: bone pain, arthralgia, bronchitis (cough), sinusitis, chest pain, peripheral edema, minor transient hypocalcemia and hypophosphatemia.

Contraindications

Hypersensitivity to the drug.
- Hypocalcemia.
- Severe renal dysfunction (creatinine clearance less than 30 ml / min).
- Pregnancy and breastfeeding (lactation).
- Children's age up to 18 years.

Carefully:

When erosive and ulcerative lesions of the mucous membrane of the gastrointestinal tract (including in history).
- In patients with impaired esophageal function in history (such as stricture or achlasia).

Drug interactions

Drugs containing polyvalent cations, such as calcium, magnesium, iron, and aluminum, can reduce the absorption of the drug.

No clinically significant interactions with non-steroidal anti-inflammatory drugs (including aspirin), blockers of H 2 histamine receptors, proton pump inhibitors, antatsidnymilekarstvennymi agents, calcium channel blockers slow, beta-blockers, thiazide diuretics, corticosteroids, anticoagulant, anticonvulsant drugs cardiac glycosides.

Compatible with hormonal replacement drugs.

Pregnancy and Lactation

Contraindicated in pregnancy and during breastfeeding.

Special instructions

Before starting drug therapy, it is necessary to correct hypocalcemia, as well as other pathologies affecting bone and mineral metabolism (for example, dysfunction of the parathyroid glands, vitamin D deficiency). In case of insufficient intake of calcium and vitamin D with food, their additional intake is necessary.

The use of a number of bisphosphonates is accompanied by esophagitis and ulcerative lesions of the esophagus. Therefore, patients should strictly follow the instructions for dosing.

Patients who are not able to stay upright for 30 minutes after taking the drug, as well as patients with an impaired esophageal function (such as a stricture or achlasia) need to use risedronic acid with extreme caution due to the limited clinical experience of using it in such patients. The physician prescribing the treatment must emphasize the importance of following the instructions for dosing regimen in such patients.

Overdosage

Symptoms (potentially possible): hypocalcemia.
- Treatment: milk intake, the appointment of antacids containing magnesium, calcium or aluminum.With a significant overdose, gastric lavage is recommended to remove unabsorbed drug.
