Clinical Pharmacology

In structure and action, salbutamol is close to other beta2-adrenergic stimulants. It has a bronchodilatory and tocolytic (relaxing uterus muscles) action. In therapeutic doses, it usually does not cause tachycardia (palpitations) and changes in blood pressure.

Indications

Treatment and prevention of reversible airway obstruction in:

- bronchial asthma (including nighttime attacks of bronchial asthma);

- chronic obstructive bronchitis;

- emphysema and other diseases of the respiratory system, accompanied by the occurrence of bronchospasm.

Composition

1 tablet contains 6 mg of salbutamol, theophylline 200 mg.

Dosage and Administration

Combipack is taken orally after a meal with water. The tablet must not be crushed, chewed or dissolved in water. Adults are prescribed 1 tablet 2 times a day.
The maximum daily dose of theophylline should not exceed 12-14 mg per kg of body weight, but not more than 1.2 g. The maximum daily dose of salbutamol is 32 mg.
The optimal doses of salbutamol and the duration of the drug are selected by the doctor individually, taking into account the clinical picture of bronchial obstruction and the patient's body weight (starting with the minimum dose of salbutamol), individual tolerance of the drug, concomitant diseases, and the simultaneous administration of other drugs. The duration of treatment is on average 1-3 months. With good tolerance and efficacy, a longer prescription period is possible.

Adverse reactions

When using Combipack, the following side effects may occur.
From the nervous system: dizziness, headache, insomnia, nervous tension, agitation, anxiety, irritability, convulsions, tremors.
From the side of the cardiovascular system: palpitations, tachycardia, arrhythmia, cardialgia, extrasystole, lowering blood pressure (BP), collapse, myocardial ischemia, increased frequency of angina attacks, feeling of "rushes" of blood to the skin of the face.
From the respiratory system: tachypnea, pulmonary edema.
On the part of the digestive system: a feeling of heaviness in the stomach, gastralgia, nausea, vomiting, dry mouth, gastroesophageal reflux, heartburn, exacerbation of peptic ulcer disease, diarrhea, with prolonged use - decreased appetite.
Allergic reactions: skin rash, itching, angioedema.
Laboratory indicators: albuminuria, hematuria, hyperglycemia, an increase in the concentration of free fatty acids in the blood, hypokalemia.
Others: increased sweating.
Side effects decrease with a decrease in the dose of the drug or disappear on their own with continued treatment for the first 2-3 weeks of therapy.
The literature describes the side effects of the monopreparations that are part of Combipack (theophylline and salbutamol) in case of overdose: allergic reactions (urticaria, erythomatous rashes, angioedema), urinary retention, neuropsychiatric disorders, including psychomotor agitation, tremor of the fingers , anxiety, hallucinations, photophobia, convulsions.
The combined prolonged-release drug Combipack compares favorably with monopreparations, because, due to the slow release of active substances, multidirectional action in some indicators (for example, urinary retention with salbutamol and the diuretic effect of theophylline), undesirable side reactions inherent in monopreparations are largely leveled.

Special instructions

The effectiveness of theophylline may be diminished in smokers.
Exercise caution when consuming large amounts of caffeinated foods or drinks during treatment.
Taking into account the profile of side effects, it is not recommended to drive vehicles and engage in other potentially hazardous activities that require increased concentration of attention and speed of psychomotor reactions when taking the drug.
Combipack is not intended for the relief of emergency conditions. Treatment with prolonged forms is carried out with periodic monitoring of the concentration of theophylline in the blood.
Sustained-release drugs greatly reduce the likelihood of dose-dependent side effects.
