Clinical Pharmacology
The combined drug has analgesic, antipyretic and anti-inflammatory effects.
Pharmacokinetics
Indications
Moderately or mild pain, of various genesis:
- headache (including migraine),
- toothache, neuralgia,
- muscle pain (myalgia),
- joint pains (arthralgia), radicular syndrome,
- menstrual pains (algomenorrhea).
Feverish syndrome with colds and other infectious and inflammatory diseases (with the established diagnosis).
Composition
1 tablet contains:
active substances: metamizole sodium 500 mg, caffeine 50 mg, thiamine hydrochloride 38.75 mg;
Excipients: microcrystalline cellulose 45.75 mg, wheat starch 11.00 mg, gelatin 4.00 mg, colloidal silicon dioxide 2.50 mg, talc 12.00 mg, magnesium stearate 6.00 mg.

Dosage and Administration
The drug is taken orally after eating 1 tab. 2-3 times in the morning.
Adults and adolescents aged 16 years and older: the maximum single dose - 2 tab., The maximum daily dose - 6 tab. for no more than 1-2 days.
Children aged 12 to 16 years: 1/2 tab. or 1 tab. 2-3 times / day. The maximum daily dose - 4 tab.
The duration of treatment is no more than 5 days when prescribed as an anesthetic and 3 days as an antipyretic.
If symptoms of the disease persist or worsen, you should consult a doctor.
Increasing the daily dose of the drug or the duration of treatment is possible only under medical supervision.
Adverse reactions
From the gastrointestinal tract: loss of appetite, nausea, vomiting, cholestasis, jaundice;
From the side of blood-forming organs: thrombocytopenia, leukopenia, agranulocytosis, hemolytic anemia, purpura;
From the side of the central nervous system: insomnia, dizziness, irritability;
Since the cardiovascular system: feeling of heartbeat, heart rhythm disturbance;
Allergic reactions: skin rash, itching, redness, angioedema, anaphylactic shock; bronchospasm.
All side effects should be reported to your doctor.
Contraindications
With care - abnormal liver function and / or kidneys.
Drug interactions
The simultaneous use of metamizole sodium with other non-narcotic analgesics, nonsteroidal anti-inflammatory drugs can lead to mutual enhancement of toxic effects.
Tricyclic antidepressants, oral contraceptives, allopurinol violate the metabolism of metamizole sodium in the liver and increase its toxicity.
Barbiturates, phenylbutazone (butadiene) and other inducers of liver microsomal enzymes weaken the effect of metamizole sodium. The simultaneous use of metamizole sodium with cyclosporine reduces the concentration of the latter in the blood.
Sedatives and tranquilizers enhance the analgesic effect of metamizole sodium.
Pregnancy and Lactation
Contraindicated in pregnancy and lactation.
Special instructions
Excessive use of caffeine-containing products (tea, coffee) during treatment may cause overdose symptoms.
With caution and under medical supervision used in patients with impaired liver or kidney function.
The simultaneous use of the drug with alcohol-containing beverages is not recommended.
Patients with atopic asthma, hay fever, have an increased risk of hypersensitivity reactions.
Influence on ability to drive vehicles and work with mechanisms
Care must be taken when driving and working with mechanisms that require increased concentration and psychomotor speed, since dizziness may develop when taking the drug.
Overdosage
Symptoms: nausea and vomiting sometimes with blood, stomach pain, mental and motor agitation, tachycardia, cardiac rhythm disturbances, decrease or increase in diuresis, convulsions, anxiety, confusion, delirium, dehydration, hyperthermia, headache, increased tactile or painful sensation tremor or muscle twitching; tinnitus, epileptic seizures (in acute overdose - tonic-clonic).
Treatment: Seek medical attention immediately. The victim should make a gastric lavage, appoint adsorbents (for example, Activated charcoal), laxative drugs.
If necessary, symptomatic treatment is performed (for hemorrhagic gastritis, the administration of antacid drugs and gastric lavage with ice-cold 0.9% sodium chloride solution; maintenance of pulmonary ventilation and oxygenation; with epileptic seizures, intravenous diazepam, phenobarbital or phenytoin; maintaining fluid and salt balance).
