Clinical Pharmacology
Heparin is a direct acting anticoagulant. It slows down the formation of fibrin, is a physiological anticoagulant that potentiates the ability of antithrombin III to inhibit activated coagulation factors IXa, Xa, XIa, XIIa, and in high concentrations - the activity of thrombin. Especially important is the ability to inhibit activated factor X, which is involved in the internal and external coagulation system (this action is shown against the background of much smaller doses of heparin than is required to suppress the activity of thrombin, contributing to the formation of fibrin from fibrinogen, which justifies the possibility of subcutaneous administration of small doses of heparin for the prevention of venous thrombosis and large doses - for treatment).
Heparin is not capable of dissolving a blood clot (it is not a fibrinolytic), but it can reduce the size of a blood clot, stopping its increase, and in this case a part of the blood clot dissolves under the action of natural fibrinolytic enzymes.
Heparin ointment has anti-inflammatory, antithrombotic, local anesthetic and anticoagulant action.
Gradually released from the ointment heparin reduces the inflammatory process and has an antithrombotic effect, promotes the resorption of existing ones and prevents the formation of new blood clots.
Benzyl nicotinate expands the superficial vessels, promoting better heparin absorption.
Local anesthetic benzocaine reduces the severity of pain.
Indications
- thrombophlebitis of the extremities (mainly superficial);
- Thrombosis of hemorrhoidal veins;
- posttraumatic hematomas;
- phlebitis after i / v injection.
Composition
1 g ointment contains:
sodium heparin - 100 U
benzocaine - 40 mg;
benzyl nicotinate - 0.8 mg

Dosage and Administration
Rectally. The suppository is introduced into the rectum after spontaneous bowel movements or cleansing enemas. Apply 1 suppository 2 times a day. The duration of the course of treatment with Nigepan® is 10-14 days.
