Clinical Pharmacology
Savzmofarm - a combined drug with a pronounced antispasmodic and analgesic effect. Metamizole sodium has a pronounced analgesic and antipyretic effect in combination with a less pronounced anti-inflammatory and antispasmodic activity. Its action is due to the inhibition of the synthesis of prostaglandins and endogenous algogens, an increase in the excitability threshold in the thalamus, an effect on the hypothalamus and the formation of endogenous pyrogens. Fenpiverinium bromide has a moderate m-holinoblokiruyuschee effect, reduces the tone and motility of the smooth muscles of the stomach, intestines, bile and urinary tract. Pitofenon has a papaverine-like effect on vascular smooth muscles and internal organs with a pronounced antispasmodic effect.
Indications
For the short-term symptomatic treatment of acute pain of a strong and moderate degree with spasms of smooth muscles of internal organs: stomach and intestinal colic; renal colic with kidney disease; spastic dyskinesia of the biliary tract; algomenorrhea
Composition
1 ml of the drug: active ingredients: metamizole sodium - 500 mg, pitofenone hydrochloride - 2.0 mg, fenpiverinium bromide - 0.02 mg; excipients: water for injection - up to 1 ml.

Dosage and Administration
Inside: adults and teens over 15 years: 1-2 pills 4 times a day, without chewing, and washing it down with a small amount of liquid.
Children 12-14 years: single dose -1 tablet, maximum daily dose - 6 pills (1.5 pills 4 times a day),
8-11 years old - 0.5 pills, the maximum daily dose - 4 pills (1 tablet 4 times a day), 5-7 years - 0.5 pills, the maximum daily dose - 2 pills (0.5 pills 4 times a day).
Adverse reactions

Contraindications
- agranulocytosis,
- leukopenia,
- thrombocytopenia,
- renal impairment: oliguria, anuria, proteinuria, interstitial nephritis,
- bronchospasm
- decrease in blood pressure
- gastralgia,
- urine staining in red.
Very rarely - anticholinergic effects (dry mouth, decreased sweating, accommodation paresis, tachycardia, urinary retention).
- hypersensitivity to pyrazolone derivatives (butadion, tribuzone), to other components of the drug;
- severe impaired liver or kidney function;
- the genetic absence of glucose-6-phosphate dehydrogenase;
- tachyarrhythmia;
- angle-closure glaucoma;
- prostatic hypertrophy with a tendency to retain urine;
- gastrointestinal obstruction;
- megacolon;
- diseases of the blood system;
- collaptoid states;
- children under 5 years old.
Drug interactions
The simultaneous use of Spazgan with other non-narcotic analgesics can lead to mutual enhancement of toxic effects.
Tricyclic antidepressants, oral contraceptives, allopurinol violate the metabolism of metamizol in the liver and increase its toxicity.
Barbiturates, phenylbutazone and other inducers of microsomal liver enzymes weaken the effect of metamizol. Simultaneous use with cyclosporine reduces the level of the latter in the blood. Sedatives and tranquilizers enhance the anesthetic effect of Spazgan.
If necessary, the simultaneous use of these and other drugs should consult with your doctor.
Pregnancy and Lactation
Contraindicated in pregnancy (especially the first trimester and the last 6 weeks), as well as during lactation.
Special instructions
During the period of drug treatment can not take alcohol. During treatment, it is not recommended to drive vehicles and engage in potentially hazardous activities that require a quick physical and mental response.
In the process of treatment is necessary to control the content of leukocytes in the peripheral blood.
If you suspect agranulocytosis or in the presence of thrombocytopenia, you must stop taking the drug. Use in nursing mothers requires cessation of breastfeeding.
Parenteral administration should be used only in cases where ingestion is impossible (or absorption from the gastrointestinal tract is impaired).
Special care is required with the introduction of 2 ml of solution and more (the risk of a sharp drop in blood pressure). IV the injection should be done slowly, in the supine position and under the control of blood pressure, heart rate and BH.
Overdosage
In case of drug overdose, the following symptoms may occur: vomiting, feeling of dry mouth, changing sweating, disturbed accommodation, lowering blood pressure, drowsiness, confusion, impaired liver and kidney function, convulsions.
Treatment: gastric lavage, the appointment of Activated charcoal, symptomatic therapy.
