Clinical Pharmacology

Hypoteph is a combination drug for treating hypertension in patients over 45 years of age, with less severe side effects than in monotherapy, its constituent components, due to lower doses.

The antihypertensive effect is achieved by simultaneously affecting most of the stages that cause the development of arterial hypertension, leading to vascular relaxation (enalapril, vinpocetine), a decrease in the amount of fluid in the body (indapamide), and normalization of cardiac activity (metoprolol). The drug prevents the increase in cerebral vascular tone caused by renal hypertension, has a therapeutic effect, including with ischemic brain damage - a condition characteristic of a very wide range of diseases, as well as for elderly people. The introduction of Vinpocetine into the combination helps to limit the development of lipid peroxidation, maintain the oxygen partial pressure in the brain, has a decongestant effect and improves mental activity during arterial hypertension in combination with cerebral ischemia.

Indications

Composition

1 tablet contains vinpocetine 2.5 mg, indapamide 0.75 mg, metoprolol tartrate 25 mg, enalapril maleate 5 mg;

Dosage and Administration

Inside before meals, 1 tablet 1 time per day (in the morning). With a lack of effectiveness and good tolerability, the dose can be increased to 2-3 tab. per day, with a uniform distribution of time between doses.

The therapeutic effect is achieved within two to three weeks of therapy.

Adverse reactions

Laboratory indicators: very rarely - hypercalcemia; unspecified frequency - an increase in the QT interval on the ECG, an increase in the concentration of uric acid and glucose in the blood, an increase in the activity of liver transaminases, a decrease in the potassium content, and the development of hypokalemia, which is especially significant for patients in the compensatory group (the probability and severity of this effect is low) .
Metoprolol:
From the side of blood-forming organs: very rarely - thrombocytopenia, leukopenia.
Since the cardiovascular system: often - sinus bradycardia, postural disorders (very rarely accompanied by fainting), "cooling" of the limbs, palpitations; infrequently - a temporary increase in symptoms of heart failure, AV block I degree; cardiogenic shock in patients with acute myocardial infarction, peripheral edema, pain in the region of the heart; rarely - gangrene in patients with previous severe disorders of the peripheral circulation.

Contraindications

Vinpocetine:
- hypersensitivity to vinpocetine;
- acute phase of hemorrhagic stroke, severe coronary artery disease, severe cardiac arrhythmias;
- pregnancy, breastfeeding period;
- age up to 18 years.
Metoprolol:
- Hypersensitivity to metoprolol;
- cardiogenic shock;
- AV blockade II and III degree (without artificial pacemaker);

Vinpocetine:

Pregnancy and Lactation

The use of Hypoteph during pregnancy is contraindicated. The active components of the drug Hypotef: metoprolol, enalapril, vinpocetine pass through the placental barrier, penetrate into breast milk.
