Clinical Pharmacology
Alflutop regulates the metabolism in cartilage tissue. The composition of the extract of marine fish includes glycosaminoglycans: hyaluronic acid, chondroitin-6-sulfate, chondroitin-4-sulfate, dermatan sulfate, keratan sulfate; amino acids; polypeptides (with a molecular weight of 50,000 daltons); macro- and microelements: sodium, potassium, calcium, magnesium, iron, zinc, copper.
Alflutop has a chondroprotective effect based on the inhibition of the activity of hyaluronidase and other enzymes that are involved in the destruction of the intercellular matrix and the normalization of the biosynthesis of hyaluronic acid and type II collagen. These effects are synergistic and determine the activation of cartilage structure restoration processes and prevent the destruction of the macromolecular structures of normal cartilage tissues.
Alflutop, restoring homeostasis in the joint, inhibits the biosynthesis of inflammatory mediators, including proinflammatory cytokines. As a result, it has a pronounced anti-inflammatory and analgesic effect, which is manifested at 8-10 days of treatment.
The proteoglycans that make up the drug have a trophic effect and have a substitute effect, significantly increasing the MRI of hydrophilicity, height of cartilage and bone homogeneity.
In cases of gastrointestinal diseases, Alflutop has a gastroprotective effect. The drug restores the synthesis of prostaglandins, reduces the time of epithelialization of ulcerative and erosive defects in NSAID gastropathies on the background of anti-ulcer therapy. It can serve as a drug of choice in patients with advanced NSAID gastropathy, if it is necessary to continue analgesic and anti-inflammatory therapy.
Indications
- primary and secondary osteoarthritis of various localization (including coxarthrosis, gonarthrosis, arthrosis of small joints);
- spondylosis;
- osteochondrosis;
- periarthritis;
- degenerative-dystrophic lesions of the joints on the background of metabolic osteopathy;
- the recovery period after surgery on the joints;
- fibromyalgia;
- paradontopathy.
Composition
1 ml of p-ra contains a standardized, defatted and deproteinized extract from 4 species of marine fish 10 mg (active substance),

Dosage and Administration
The drug is prescribed for adults.
Withpolyosteoarthrosis and osteochondrosis Alflutop is administered deeply IM at 10 mg (1 ml) per day. The course of treatment is 20 injections.
Withprimary lesion of large joints the drug is administered into the joint by 10-20 mg (1-2 ml) with an interval of 3-4 days between injections. The course of treatment is 5-6 injections into each joint.
More effectively combined use of intramuscular and intraarticular injections.
If necessary, the treatment should be carried out after 3 months, followed by 6 months.
Adverse reactions
Seldom: pruritic dermatitis, reddening of the skin, burning sensation at the injection site, short-term myalgias.
In some cases: transient increase in pain syndrome (with intra-articular administration due to the activation of metabolism and blood circulation intensification).
Contraindications
- pregnancy;
- lactation (breastfeeding).
It is not recommended the appointment of the drug in children and adolescents due to the lack of clinical data confirming the safety and efficacy of its use in this age group.

Special instructions
There is evidence of the effectiveness of intramuscular paravertebral administration of Alflutop in the treatment of patients with spinal osteochondrosis with radicular and non-crack pain syndromes during exacerbation and in partial or complete remission.
Use in Pediatrics
It is not recommended the appointment of the drug in children and adolescents due to the lack of clinical data confirming the safety and efficacy of its use in this age group.
Overdosage
Symptoms: In susceptible patients, allergic reactions are possible, sometimes severe.
Treatment: the abolition of the drug, conducting symptomatic therapy.
