Clinical Pharmacology

Chymotrypsin is a proteolytic enzyme preparation derived from the pancreas of cattle.

Hydrolyzes proteins and peptones to form relatively low molecular weight peptides, cleaves bonds formed by aromatic amino acid residues (tyrosine, tryptophan, phenylalanine, methionine).

In medical practice, the use of chymotrypsin is based on the specific peculiarities of cleaving necrotic tissue and fibrinous formations, thinning viscous secrets and exudates, and, when administered intramuscularly, have an anti-inflammatory effect.

Indications

  • Suppressive diseases of the respiratory system: tracheitis, bronchitis, bronchiectasis, pneumonia, lung abscesses, atelectasis, exudative pleurisy, bronchial asthma.
  • In the treatment of burns, pressure sores, purulent wounds, thrombophlebitis.
  • In ophthalmology: with fresh extensive thrombosis of the central retinal vein, acute obstruction of the central retinal artery, opacities of the vitreous body of traumatic and inflammatory origin, cataract extraction.
  • In otolaryngology: with purulent sinusitis, subacute laryngotracheitis, after tracheostomy to facilitate the removal of thick viscous exudate, with acute, subacute, purulent, chronic otitis media and eustachitis with viscous exudate.

Dosage and Administration

For intramuscular administration, immediately before use, 0.005 g of the drug is dissolved in 1-2 ml of sterile 0.9% sodium chloride solution or 0.5-2% procaine solution.

The course of treatment is 6-15 injections.

For respiratory diseases: the drug is used intramuscularly in doses of 5-10 mg per day for 10-12 days, it may be combined with the use of chymotrypsin aerosol in 5% aqueous solution in an amount of 3-4 ml. After 7-10 days, the treatment can be repeated.

In the treatment of burns and bedsores: Chymotrypsin in a dose of 20 mg is dissolved in 20 ml of a 0.25% solution of procaine and a thin needle is injected under a scab with several injections.

In the treatment of purulent wounds: purulent wound surfaces impose sterile wipes moistened with a solution of 0.025-0.05 g of chymotrypsin in 10-50 ml of a 025% procaine solution for 8 hours. For the treatment of purulent wounds, intramuscular injections of the drug are combined with local treatment of the wound with tampons moistened with a 5% aqueous solution of chymotrypsin.

With thrombophlebitis: the drug is administered intramuscularly in a dose of 5–10 mg daily for 7–10 days.

For cataract extraction: Chymotrypsin at a dilution of 1: 5000 is introduced into the posterior chamber of the eye, followed by washing the anterior chamber of the eye with a 0.9% sodium chloride solution 4 minutes after drug administration.

In the treatment of thrombosis of the central vein of the retina, acute obstruction of the central artery of the retina: Chymotrypsin is administered subconjunctivally at 0.2 ml of a 5% solution prepared in 1% procaine solution, 1-2 times a week.

With sinusitis: the drug is introduced into the corresponding cavity in the amount of 5-10 mg in 3-5 ml of 0.9% sodium chloride solution after puncture and washing the cavity.

With otitis media: drop in the ear with 0.5-1 ml of 0.1% solution of chymotrypsin in 0.9% sodium chloride solution.

When micro-operations on the ear to soften fibrinous formations in the middle ear: during surgery, a 0.1% chymotrypsin solution is injected into the cavity. Simultaneously, chymotrypsin should be administered intramuscularly at 2.5-5 mg 1-2 times a day.

The drug is diluted in 1-2 ml of a 0.25-0.5% solution of novocaine or 0.9% sodium chloride solution.

Adverse reactions

Maybe: allergic reactions.

Contraindications

It should not be injected into the bleeding cavities, applied to the ulcerated surfaces of malignant tumors in order to avoid the spread of the malignant process.

Carefully used in empyema tuberculosis ethnology (resorption of exudate may in some cases contribute to the development of bronchopleural fistula).
