Geptral is a medication that is used to treat liver disorders and depression. Several studies also suggests that the drug can be used to treat other mental disorders as well. The active ingredient of Geptral is S-Adenosylmethionine (Ademetionine).
Ademetionine is the methyl donor to numerous and diverse acceptor molecules including DNA, proteins, phospholipids and catechol- and indoleamines.
The therapeutic effect of Geptral in some depressive illnesses has led to new nontoxic treatment strategies. There have been encouraging pilot studies of the use of Ademetionine in patients with Parkinson’s disease, epilepsy and Alzheimer’s dementia. Furthermore, impaired C-I metabolism and Heptral synthesis has been proposed to be a common pathogenic mechanism in Alzheimer’s dementia, Down’s syndrome and HIV infection, thus lending further support for clinical trials with methyl donors in these conditions.
Convincing evidence from studies of inborn errors of methyl group metabolism have shown that methylation is important in maintaining the integrity of the myelin sheath. Correction of the methyl group deficiency with methyl group donors, including oral Ademetionine treatment, can halt or even reverse the demyelination process. Heptral appears to influence monoamine neurotransmitter metabolism and receptors, which may be the putative mechanism of action in the treatment of depression.
Clinical trials to date with Heptral have shown it to be a relatively safe compound, with no serious adverse reactions observed, and it is devoid of any hepatotoxic effects. This is probably related to the fact that it is a natural ubiquitous metabolite present in all organisms. The full clinical potential of Ademetionine in some neurological disorders remains to be explored.

Dosage and Administration
During the period of maintenance therapy, a daily dose of 800–1600 mg (2–4 pills) is recommended. The duration of maintenance therapy is on average 2-4 weeks. pills should be swallowed whole, without chewing. To improve the therapeutic effect of pills, it is recommended to take between meals.
Adverse reactions
Due to the fact that the active substance of the drug has an acidic pH, some patients may experience discomfort in the epigastric region when using pills. In any case, these feelings are short-lived, they are not pronounced and should not be a reason for stopping the course of treatment.
Contraindications
Hypersensitivity.
Drug interactions
No known interactions with other drugs were observed.
Pregnancy and Lactation
Not recommended in the I-II trimester of pregnancy and lactation.
Special instructions
Given the tonic effect of the drug, it is not recommended to take before bedtime. Oral use of the drug in patients with cirrhosis associated with hyperazotemia should be carried out under the supervision of a physician with controlled nitrogen levels. A pill of the drug Geptor must be removed from the packaging immediately before taking. The pills of the drug are coated with a special coating that dissolves only in the intestines, so that ademetionin is released in the duodenum.
