Clinical Pharmacology

Bisacodyl is a laxative.

Pharmacodynamics

Is a prodrugwhen taken orally in an alkaline environment, hydrolysis occurs with the formation of a substance that has an irritating effect on the gastrointestinal mucosa (increases the secretion of mucus in the colon, accelerates and increases its motility). When taken orally, it acts after 6–10 hours; when taken before bedtime, after 8–12 hours.

Rectal use increases the secretion of mucus in the large intestine, accelerates and strengthens its motility. The action is due to direct stimulation of the nerve endings in the mucous membrane of the large intestine.

Indications

  • Constipation due to hypotension of the colon;
  • bowel cleansing in terms of preoperative preparation;
  • preparation for instrumental and radiological studies.

Dosage and Administration

For constipation if the doctor has not appointed otherwise, then it should be used: adults and children over 10 years old - 1 suppository per day. The suppository is inserted into the rectum with the pointed end forward. The action takes about 30 minutes.

In preparation for research, pre- and postoperative treatment, medical conditions, which require relief of bowel movements Dulcolax must be used under medical supervision.

It is recommended in combination with pills to achieve complete emptying. In addition to the pills taken the day before, one suppository is recommended in the morning.

Adverse reactions

From the digestive system: possible spastic abdominal pain, diarrhea.

Very rare: hypersensitivity reactions with skin manifestations, anaphylaxis has been reported extremely rarely. The use of suppositories may be accompanied by pain and local irritation, especially during anal fissure or ulcerative proctitis.

Contraindications

  • Hypersensitivity to the drug or its components;
  • intestinal obstruction;
  • obstructive bowel disease;
  • acute diseases of the abdominal organs, including appendicitis;
  • acute inflammatory bowel disease;
  • severe abdominal pain associated with nausea and vomiting, which may be a sign of a more serious condition;
  • severe dehydration;
  • children under 10 years old.

Drug interactions

Diuretics or glucocorticosteroids increase the risk of electrolyte imbalance (hypokalemia) when taking high doses of Dulcolax.

Disruption of electrolyte balance (hypokalemia) enhances the action of cardiac glycosides.

Pregnancy and Lactation

Data on reliable and well-controlled studies in pregnant women are not available. Long experience of use did not reveal the negative effect of the drug on pregnancy. Bisacodyl does not penetrate into breast milk. However, as with the use of other drugs, it is necessary to prescribe the drug with caution during pregnancy, especially in the first trimester, and during the breastfeeding period.

Special instructions

Do not use the drug for a long time. If there is a need for daily intake of laxatives, you should consult a doctor to identify such causes. Long-term use can lead to fluid loss, electrolyte imbalance and hypokalemia.

Dizziness and fainting were observed in patients taking Dulcolax. The analysis showed that these cases are associated with fainting during bowel movements (or fainting caused by stress during defecation), or with a vasovagal response to abdominal pain, which may be associated with constipation, which forced patients to take laxatives, and are not necessarily associated with taking the drug .

Children should not take bisacodyl without the advice of a doctor.

Overdosage

Symptoms: overdose may cause diarrhea, dehydration, abdominal cramps, impaired water and electrolyte balance, hypokalemia. Dulcolax, like other laxatives, in chronic overdose can lead to chronic diarrhea, abdominal pain, hypokalemia, secondary aldosteronism, urolithiasis. Due to chronic abuse of laxatives, renal tubular damage, metabolic alkalosis and muscle weakness associated with hypokalemia may develop.
